Assessment Information

Some Important Information About
The Center for School Success

The Center for School Success (CSS) works with school-aged children and young adults (from 2nd grade-college) who experience learning challenges in general education classes. CSS services focus primarily on learning issues within the general classroom, so the model is not suitable for students with more complex needs.


• Students with significantly delayed cognitive functioning would benefit from specialized assessments related to their needs. Your pediatrician should be able to refer you to a developmental evaluation facility in your area.

• CSS does not address significant emotional or behavioral conditions. If this is your primary concern, please contact a mental health professional.

CSS assessments are tailored to each individual student. They are designed to identify neurodevelopmental strengths and weaknesses and to link the strengths and weaknesses to academic skills. For example, to discover why a student may struggle with spelling, CSS clinicians examine memory, attention, language and sequencing. The assessment is dynamic in that CSS clinicians interact with the student during the process, talking about how the student approaches tasks and experimenting with possible teaching techniques strategies.
     
Because of the dynamic nature of the CSS assessment, neither IQ nor standardized achievement testing are provided. Labels are not used (e.g., "learning disabled", "ADD") in reports or explanations to parents. Consequently, CSS assessments are not designed to verify, determine or refute eligibility for special education services. Reports do include a customized learning plan that contains specific suggestions parents can use to help their child address his/her learning challenges and maximize his/her strengths. Typically, parents will want to share the learning plan with their child's teacher(s). However, CSS advises parents that a school is not obligated to use any information/suggestions listed in the report.
The fee for a CSS assessment is $3,000. The fee includes: 1) a comprehensive clinical review; 2) a day-long, customized assessment; 3) a detailed written report within 6 weeks after the assessment; 4) a follow-up visit to discuss the report; and 5) another follow up visit 4-6 months after the initial assessment to monitor progress. CSS does not bill schools or insurance directly, but can provide coding language to assist you in seeking reimbursement for portions of the assessment. CSS offers a tiered fee scale so that its assessments can be accessible to as many families as possible (please refer to enclosed explanation).


CSS assessments are conducted in October, January, March and May.
In order to process your request as quickly as possible, we ask that you note the following procedures:


1. The attached Referral Form is used to determine whether CSS services can appropriately meet the needs of your child. We must have a completed Referral Form to process your request.

2.       Send only the completed Referral Form by mail to: The Center for School Success, 79 E. Wilder Road, West Lebanon, NH 03784 or by fax to: (603) 298-6703. Please do not send any other information, such as medical records or previous assessment data at this time.
3.   The Center for School Success will contact you within two weeks of receiving your form to let you know if CSS services are appropriate to meet your child’s needs.                                                                               8/07
